Please note if you cannot do the test within this time that there is a rate of 200 EUR (excluding VAT) for expenses.

By sending this form I accept the privacy policy of YourPrevention™, a brand of BioAging & BioLabs GmbH. I confirm the compensation of all cost of the IST™. I authorize YourPrevention™ to send me the IST™ test. I confirm the compensation of 200 EUR plus VAT of expenses if the test has not been finished within 14 days upon receipt. I furthermore authorize YourPrevention™ sending me the secure SSL Login for the online questionnaire and the HRV protocol to the indicated E-Mail address. I hereby release YourPrevention™ from the medical confidentiality to forward the IST™ report to the indicated physician or coach.
